Eligibility:
- Adults 60 years or older (no income requirement), and their spouse/partner of any age.
- Adults 18 to 59 years old with disabilities (must reside in special/senior housing with its own meal site).
- A government-issued identification document will be required to verify age for eligibility.

Application Procedure:
- Applicants can either submit their application online at: Santa Clara CHAMPSS Application
And visit one of the Self-Help for the Elderly at Sunnyvale Senior Center.
- Processing the CHAMPSS application typically requires 2 to 4 weeks.
After processing, your designated senior center will contact you to schedule an in-person appointment to verify your eligibility.
- A government-issued identification document is required to verify your age.
If you are an existing Senior Nutrition Program (SNP) participant, bring your gold card (participant registration card).
- Once your application has been verified and completed, you will receive two cards:
1) A gold card with a barcode to be used at Santa Clara County SNP meals sites; and
2) a CHAMPSS card to for use at CHAMPSS restaurants in Santa Clara County.
- Note: If you are a current SNP gold card holder in Santa Clara County, you can register for CHAMPSS:
- Go to the SNP Registration Form
- Select Another Nutrition Site Only
- For Nutrition Site Name to Add, select "CHAMPSS" in the dropdown option.
- Submit form
- Contact the senior center (415) 677-7610 after submitting the form.

Usage policies:
- The Santa Clara County CHAMPSS card is only accepted at the participating restaurants. Participants can dine in during the restaurant’s program lunch hours. See restaurant menu for meal service times.
- Participants can only receive one meal per day, either at a CHAMPSS restaurant or at a SNP congregate meal site.
- Take-out orders are not allowed. This is a dine-in only program.
- A maximum of 5 meals per week can be added, either through online (credit card) or in-person at Self-Help for the Elderly Community Center (cash or check).
- No cash or check transactions will be permitted at the restaurant.
- Meals added on or after July 1, 2024, will be valid until June 30, 2025.
